CareS Health Services handles personal health information in accordance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA). We are committed to protecting the privacy and confidentiality of our clients, their families, our partner facilities, our donors, our staff and job applicants.

What we collect

We collect only the information needed to provide and coordinate care, respond to inquiries and staffing requests, process job applications, and process donations and issue tax receipts. Information submitted through this website includes the details you enter in our forms.

How we use and share it

Personal health information is used to deliver care and may be shared with your health care team (such as your physician or care facility) as permitted by law. We do not sell or rent personal information.

Safeguards

We use administrative, technical and physical safeguards to protect information. Website forms are transmitted securely, and access to submissions is limited to authorized staff.

You may request access to or correction of your personal health information, or withdraw consent, by contacting our Privacy Officer at info@careshealth.ca. You may also contact the Information and Privacy Commissioner of Ontario at www.ipc.on.ca.
